Match Change Request Form

All Match Change Requests must be submitted to the OVPL no later than 10 days prior to the originally scheduled match date.

Before submission, all involved clubs must agree to the proposed change in Date, Time, and/or Venue. Submission of a request does not constitute approval and all changes remain subject to formal league review and approval.

Requests submitted within 72 hours of the scheduled match will be automatically declined unless the league determines the circumstance to be extreme or unavoidable in nature.

Requests occurring less than 24 hours prior to a scheduled match will be declined and subject to league review to determine whether the match will be rescheduled or recorded as a 3-0 forfeit.

The OVPL reserves the right to deny any request that may negatively impact competition integrity, player safety, referee operations, travel logistics, or overall league scheduling.
